Polyendocrine Metabolic Ovarian Syndrome (PMOS) is the new name for the condition formerly known as polycystic ovary syndrome (PCOS). The name changed in May 2026 to better reflect that the condition involves much more than the ovaries.

PMOS is a complex hormonal and metabolic condition that can affect menstrual cycles, ovulation, androgen levels, metabolism, fertility, skin and hair, sleep, cardiovascular health, and overall quality of life.
For years, the term “polycystic ovary syndrome” contributed to the idea that ovarian cysts were the defining feature of the condition. They are not. The ovaries are one part of a much larger picture.
What Is PMOS?
PMOS occurs when several hormonal and reproductive features come together in a pattern that meets established diagnostic criteria. The condition can look very different from one person to another.
Some people may experience irregular or missed periods. Others may notice acne, excess facial or body hair, scalp hair thinning, difficulty with ovulation, or fertility challenges. PMOS can also be associated with insulin resistance and an increased risk of metabolic conditions such as type 2 diabetes.
Not everyone with PMOS will experience every symptom, and the presence or absence of a single symptom does not determine whether someone has the condition.
Why Did the Name Change?
The former name focused heavily on the ovaries and the appearance of multiple follicles that may be seen during an ultrasound.
The updated name, Polyendocrine Metabolic Ovarian Syndrome, recognizes the broader nature of PMOS and emphasizes its hormonal, metabolic, ovarian, and reproductive components.
The name change does not mean that the condition itself suddenly appeared or that everyone previously diagnosed with PCOS has a different disease. It reflects an effort to describe the condition more accurately.
What Are the Main Features of PMOS?
PMOS is generally diagnosed by looking for a combination of three major features:
- Problems with ovulation or menstrual cycles
- Clinical or biochemical signs of higher androgen activity
- Polycystic ovarian morphology on ultrasound, when appropriate
A person does not necessarily need to have all three.
Importantly, an ultrasound showing a particular ovarian appearance does not automatically mean someone has PMOS. Other causes of similar symptoms must also be considered before making a diagnosis.

PMOS Is More Than Fertility
Fertility is an important part of PMOS, but it is only one part.
Because PMOS can affect ovulation, some people may have difficulty becoming pregnant. However, PMOS does not mean infertility.
Some people with PMOS become pregnant without fertility treatment, while others may need assistance with ovulation or other aspects of conception. Treatment depends on the individual’s circumstances and whether pregnancy is currently desired.
PMOS care should therefore continue even when pregnancy is not a current goal.
The Metabolic Side of PMOS
The word metabolic in the new name is important.
PMOS is associated with insulin resistance and an increased risk of abnormal glucose regulation and type 2 diabetes. It can also be associated with other cardiovascular risk factors, including high blood pressure and abnormal cholesterol levels.
However, insulin resistance does not mean someone has diabetes, and having PMOS does not automatically mean a person will develop diabetes.
Metabolic health is one part of PMOS that deserves attention alongside reproductive and hormonal health.
PMOS Can Affect the Whole Person
The effects of PMOS can extend across several areas of health, including:
Reproductive health: irregular periods, ovulation problems, and fertility challenges.
Hormonal health: increased androgen activity and hormonal imbalance.
Metabolic health: insulin resistance and increased risk of type 2 diabetes.
Skin and hair: acne, excess hair growth, and scalp hair thinning.
Cardiovascular health: increased risk factors such as high blood pressure and abnormal cholesterol.
Sleep: increased likelihood of sleep-related conditions such as obstructive sleep apnea.
Mental health and quality of life: depression, anxiety, body-image concerns, and other challenges can occur alongside PMOS.
This does not mean that every person with PMOS will experience all of these issues. It means that effective care should look beyond the ovaries.
How Is PMOS Diagnosed?
There is no single blood test that can diagnose PMOS.
Healthcare professionals typically consider a person’s menstrual and reproductive history, symptoms, physical examination, laboratory testing, and sometimes ultrasound. Testing may also be used to rule out other conditions that can cause similar symptoms.
Diagnosis is therefore a process rather than the result of one abnormal hormone level or one ultrasound.
How Is PMOS Managed?
There is no single treatment that works for everyone with PMOS.
Management depends on symptoms, overall health, metabolic risks, reproductive goals, and personal preferences. Treatment may focus on regulating menstrual cycles, managing androgen-related symptoms, improving metabolic health, supporting fertility, or addressing other health concerns.
Lifestyle changes and medications may both have a role, depending on the individual.
The goal is not simply to treat one symptom. Good PMOS care looks at the whole picture.
The Bigger Picture
PMOS represents a broader way of understanding a condition that has often been reduced to periods, fertility, or the appearance of the ovaries.
It is a hormonal, metabolic, ovarian, and reproductive condition that can influence multiple areas of health.
Understanding PMOS means understanding that the ovaries are part of the story—but they are not the whole story.
